Summary

Streptococcus belongs to the family of lactobacillacae and the majority is facultative anaerobes. The facultative anaerobicstreptococci are divided into two categories:
1. _ those which produce soluble hemolysin and
2. _ those which do not produce soluble hemolysin
The first group of streptococci are called B-hemolytic streptococci, which can be further subdivided into group (a), group (b),group (c} and group (d). Itincludes most of the species associated with primary streptococcal infections in humans.
The group (a) 8-hemolytic streptococci produce various exotoxins such as streptolysin O and streptolysin S that can act asantigens. The affected individuals produce specific antibodies against streptolysin O, namely Anti-streptolysin O.Determination of these antibodies is very useful for the diagnosis of streptococcal infections and their relative effects such astheumatic fever and acute glomerulonephritis. An elevated ASO titre of more than 200 IU/ml may indicate an acutestreptococcal infection.

Reagents
1. Rhelax*-ASO reagent: A uniform suspension of polystyrene latex particles coated with streptolysin 0. The Rhelax’-ASO reagentis standardized to detect antibodies to streptolysin O in concentrations ranging from 200 IU/ml or more. Thestandardization of detection limit of RHELAX°-ASO is traceable to the International Standard for Antistreptolysin ‘O'(97/662).
2. Positive control, reactive with the Rhelax*-ASO reagent.
3. Negative control, non-reactive with the Rhelax*-ASO reagent.

PRINCIPLE
Lactate dehydrogenase catalyzes the reduction of pyruvate with NADH to form NAD. The rate of oxidation of NADH to NAD is measured as a decrease in absorbance, which is proportional to the LDH Activity in the sample.

Summary

Enteric fever occurs when pathogenic microorganisms like S. typhi, S. paratyphi A S. paratyphi B infect the human body. During the course of disease, the body responds to this antigenic stimulus by producing antibodies whose titre rises slowly in early stages, to a maxima and then slowly falls till it is undetectable. Antibodies to salmonella organisms may be detected in the patient serum from the second week after onset of infection. ® Information regarding the titres and whether or not they are rising or falling can be obtained by performing serological tests using VITAL WIDAL antigen suspensions. Usually tube titres of 1:80 and above are taken as diagnostically significant, however for endemic areas higher cut-offs may need to be established.

Reagent 
Vital widal contains ready to use coloured, smooth antigen suspensions of the bacilli; S. typhi 'O', S. typhi 'H', S. paratyphi 'AH' and S. paratyphi ® 'BH'. Vital widal reagents are versatile and standardized for use in a standard tube test procedure for the detection of S. typhi and S. paratyphi antibodies in the patient's serum.
Principle 
When the coloured, smooth suspension of attenuated Vital widal antigen suspensions are incubated with the patient serum, anti-salmonella antibodies present in the patient's serum react with the antigen suspensions to produce agglutination. Agglutination is a positive test result, indicating presence of salmonella antibodies in the patient's serum. No agglutination is a negative test result indicating absence of salmonella antibodies in the patient's serum.

The organism Salmonella typhosa is responsible for causing enteric fever or typhoid fever , which is characterized generally by very high consistent fever, loss of appetite, transitory bacteraemia, round or oval shaped ulcer on smooth peritoneal surface of Peyer's patches and solitary lymphoid follicle of ileum etc. The organism possess 'O' antigen on the cell wall and 'H' antigen on it's flegella, against which the host body produces immunospecific antibodies, to counteract the effect of corresponding antigens. On the other hand the paratyphoid fever caused by Salmonella paratyphi A or Salmonella paratyphi B is characterized by milder course of disease. These organisms also possess somatic 'O' and flagellar antigen which is termed as A(H) and B(H) respectively. The Other Organisms of Salmonella species like Salmonella typhimurium is responsible for causing food poisoning or Arizona group causing fetal infection do have similar antigenic properties.

Lipases are enzymes which hydrolyse glycerol esters oflongchain fatty acids. The enzyme and its cofactor, colipase isproduced in the pancreas, lipase being also secreted insmall amounts by the salivary glands as well as by thegastric, pulmonary and intestinal mucosa. Bile acids andcolipase form micellar complexes with the lipids and bindlipase on the substrate water interface. Determination ofLipase is used for investigation of pancreatic disorders. Inacute pancreatitis the lipase concentrations rise 2-50 foldthe upper reference limit within 4-8 hours after beginning ofabdominal pain peaking at24 hours and decreasing within 8-14 days. Elevated Lipase values can also be observed inchronic pancreatitis and obstruction ofthe pancreatic duct.

Principle:
Pancreatic Lipase catalyses the hydrolysis of Triolein, in thepresence of colipase, to form monoglycerides and fattyacids. The rate of decrease in turbidity measured at 340nm,is proportional to the lipase activity. The activities of otherlipases in the serum are inhibited by the cholic acid salts inthe reagent.

The Viral Hepatitis caused by Hepatitis B Virus is termed as “Serum Hepatitis”. 1-5% infected people act as chronic carriers of HBV Virus. Major part of the chronic carriers secrete hepatitis B surface antigen (HBsAg) into blood and other secretions of the body like saliva and vaginal fluid. These chronic carriers are potentially infectious to other seronegative people. Hepatitis B Virus belongs to a family of enveloped DNA virus, the Hepadnavirus. Related viruses in this group cause chronic hepatitis. HBsAg has been accepted as a universal and the most reliable seromarker in case of acute HBV infection due to its appearance almost 2-4 weeks before the ALT level becomes abnormal and 3-5 weeks before the onset of symptoms or jaundice as an early detection system for hepatitis infection. In most cases of HBV infection, the incubation period varies from 40 days to 6 months. Within HBVs, antigenic diversity is recognised in the surface antigens. HBsAg particles contain common “a” antigen, linked to two sets of mutually exclusive determinants, “d” or “y” and “w” or “r” giving the four main types-adw, adr, ayw and ayr.

2. Intended use
Hepalisa is designed for in-vitro qualitative detection of Hepatitis B surface antigen (HBsAg) in human serum or plasma and is used as a screening test for testing of collected blood prior to transfusion.

3. Serological markers of clinical significance for hepatitis B virus (HBV)
HBsAg : First detected during the incubation period of 6-8 weeks before the appearance of symptoms. It can appear as early as 2 weeks and generally disappears within 3-4 months after exposure. It is the most reliable & universal marker of HBV infection. In the carrier and chronic state it persists more than6 months.
Anti-HBs : Appearance may take several weeks/months after HBsAg clearance causing a ‘window’ period. Best indicator of recovery & immunity to HBV. Quantitation allows pre-vaccination screening & follow-up of the post-vaccination response.

HBeAg : Appears within 1 week after HBsAg, lasts 3-6 weeks & disappears before HBsAg clearance. Its presence indicates a highly infectious state, except in pre-core mutants. Persistence > 10 weeks suggests progression to chronic carrier state/hepatitis.

Anti-HBe : It appears before clearance of HBsAg, indicates decreasing infectivity and is a good prognosis for the resolution of infection.

Malaria is a serious, sometimes fatal, parasitic disease characterized by fever, chills, and anaemia and is caused by a parasite that is transmitted from one human to another by the bite of infected Anopheles mosquitoes. There are four kinds of malaria parasite that can infect human: Plasmodium falciparum, P. vivax, P. ovale and P. malariae. At present, malaria is diagnosed microscopically by looking for the parasites in a drop of blood. 3. Principle of the test

Malaria Ag Microlisa test is an enzyme immunoassay based on sandwich ELISA. Anti-pLDH antibodies has been coated onto the wells of the microwell plate provided. Blood sample including controls are pipetted into these wells. During the first incubation, the plasmodium species pLDH antigen from samples and coated antibody is react and after washing the enzyme conjugate (anti-pLDH antibody HRPO) is added after incubation and washing to remove all the unbounded enzyme conjugate. A substrate solution which acts on the bound enzyme is added to induce a colured reaction product. The intensity of this coloured product is directly proportional to the concentration of pLDH present in the samples
